This form is required for any student wishing to take a course offered by the Electrical & Computer Engineering Department for which they do not have credit in the Pre-requisite course/s, or need Permission because they are not in an ECE program.
If you need Permission for a non-ECE course, refer to the website of the department which offers that course to find the form and procedure used by that department.
Use 1 form per course.
By submitting this request the student accepts responsibility for keeping up with the class.
Submission of this form is equal to electronic signature.
INSTRUCTIONS FOR STUDENTS:

Complete the information on this form.
If there is no instructor currently assigned to the course please select "UG Admin" in which case the UG Admin will be notified.
If you cannot find the assigned instructor in the list, please select "Not Listed Here" and provide their email address.
A copy of your unofficial transcript (from your Student Centre - Academics section) must be submitted with this form.
The instructor will receive the request and forward it for final processing, Cc'ing you to show you that it has been authorized (permitted or denied).
After receiving an approved email from the instructor, the UG Admin will upload the Permission to Mosaic. You will be able to enroll within 2 to 3 business days. Please be patient.
